Referral

How to make a Referral?

We will take great care in carefully reviewing the referral information and learning more about each child or young person. It is important they have the time to review the information and ask for further information before they complete a moving in assessment and initial plan to see if this is the best home and how the team will be able to meet their needs. It is important that before a moving in agreement is made the child or young person’s moving in assessment is completed. We also welcome children, young people and their families as part of a visit to the home where this is in their best interest.

Once a moving in agreement is made each young person will have a named mentor from the team whose role it is, is to provide 121 opportunities for support to achieve and celebrate progress. Having a mentor does not mean the rest of the team are not on hand to support too, as everyone is there to help and support with daily living.

Referral information

It is important to us that we work closely with you to understand the needs of the child or young person who is considering whether we are the right team to care or support them. To help us all do this well we will ask for some general information in the first case before we ask for more details as we know everyone’s time is so precious.

Whenever you provide us with information about a young person, we will treat the information with the upmost respect and kindness and will look to respond to you as soon as possible. If you wish to make a referral, please complete the making a referral section below.
